Tracy House is a SW Ambulatory Care Coordinator for CHI Saint Joseph Health Partners. She received her Licensed Bachelors of Social Work from Eastern Kentucky University and has almost 30 years of experience in the medical field. Before joining the Health Partners team in 2015, Tracy worked for Tri-County Hospice as a social worker, bereavement coordinator, and volunteer coordinator. She currently serves as the President for the Parish Pastoral Counsel and has been over seeing renovation with St. Sylvester Oratory since 2014. In her leisure time, Tracy is highly involved as a grandparent, enjoys reading, and volunteers at her church.

Teresa Lucas is a RN Ambulatory Care Coordinator for CHI Saint Joseph Health Partners. She earned her BSN, RN from Eastern Kentucky University. Teresa has over 30 years of nursing experience in hospital nursing, hospital management, home health, cardiac rehabilitation, quality metrics, and case management. She has many accomplishments including the 2013 Saint Joseph London Hospitalist Intervention Program to Reduce Hospital Re-admissions (Modern Healthcare – Ranked 5^th in Most Improved Hospitals), received an Excellence in Nursing award, and serves on several committees including the Laurel County Community Lions Club as the Vice President, the Cumberland Valley USA Dance Club as the treasurer, and is a primary nurse for the Community Christian Church Healthcare Clinic for the Uninsured. In her leisure time, Teresa enjoys cycling, dancing, and volunteering.

Renee Sizemore, an Ambulatory Care Coordination Assistant, joined the CHI Saint Joseph Health Partners team in February 2017.  She has over 25 years of health care experience, serving in both clinical and administrative settings. Renee’s experience includes medical assisting, medical billing, DME billing, administrative secretary and office coordinator roles – inside the hospital and in physician and physical therapy offices. Renee is a proud mother of two teenage boys and family is her top priority. In her leisure time, Renee loves life on the lake– water skiing, tubing, and knee boarding. She also enjoys hiking, basketball and karaoke.

Kari Bargo is a RN Ambulatory Care Coordinator for CHI Saint Joseph Health Partners. She received her BSN from Lincoln Memorial University and has over 30 years of experience in pharmacy and nursing combined. Before coming to Health Partners, Kari served in Transitional Care/Case Management at Jellico Medical Center. In her leisure time, she enjoys saving stray animals and driving her ’57 Chevy.
