Hana Gilbert, BSN, RN

Hana Gilbert is an RN Ambulatory Quality Care Coordinator with CHI Saint Joseph Health Partners. Hana received her RN and BSN from the University of Kentucky. She has 10 years of experience in hospice care, public health, school health, pediatric ICU and utilization management. She also has worked on Early and Periodic Screening Diagnosis and Treatment (EPSDT), Pediatric Acute Life Support (PALS) and KIDS SMILE oral Health Training Program. In her leisure time, Hana enjoys spending time in the greater outdoors, fitness and obstacle course racing.

Tracy House

Tracy House, BSW, LSW

Tracy House is a SW Ambulatory Care Coordinator for CHI Saint Joseph Health Partners. She received her Licensed Bachelors of Social Work from Eastern Kentucky University and has almost 30 years of experience in the medical field. Before joining the Health Partners team in 2015, Tracy worked for Tri-County Hospice as a social worker, bereavement coordinator, and volunteer coordinator. She currently serves as the President for the Parish Pastoral Counsel and has been over seeing renovation with St. Sylvester Oratory since 2014. In her leisure time, Tracy is highly involved as a grandparent, enjoys reading, and volunteers at her church.

Erin Warren, RN, BSN, MBA

Erin Warren, RN, BSN, MBA

Erin started her career as a surgery nurse extern with CHI Saint Joseph Health in the summer of 2006. After graduating from nursing school, she and her family lived in Western Kentucky before moving to Louisville in 2016. She returned to CHI Saint Joseph Health Partners in the fall of 2023 as a RN Ambulatory Care Coordinator and brings a diverse perspective to our team. 

Erin has over 20 years of combined experience in marketing, nursing, physician relations, medical device sales and nurse management. She received all of her education from Western Kentucky University; including bachelor degrees in Public Relations (2001) and Nursing (2006) and Master of Business Administration (2015). 

Erin has a heart for vulnerable and underserved people groups. Since the 2010 earthquake in Port-Au-Prince, Haiti, she has made several trips to the third-world country as part of medical and surgical teams to help care for injured victims and bring healthcare to remote areas with no access to primary care. She also enjoys camp nursing and is an active member of the medical team at her church, Southeast Christian in Louisville, KY. 

Erin is passionate about helping people set goals and reach them so that they can enjoy life to the fullest. Prevention and wellness are important aspects of her personal life that carry over to her care of her patients. 

In her free time, Erin and her family enjoy spending time outdoors; running, hiking, camping and kayaking. Driven by her faith in God, she and her family often look for opportunities to help others and regularly volunteer at their church.
