To further coordinate quality patient care and assist with documentation, the KMG Quality Committee is launching a new initiative. According to Charlie Powell, KMG will be implementing use of the Healthcare Quality Patient Assessment Form (HQPAF) program to promote early detection and ongoing assessment of chronic conditions. Powell said, “The HQPAF program is simply one more historical look at our patients to ensure we are accurately identifying and reporting chronic conditions, in an effort to provide the best possible quality care.”
Training for HQPAF will begin this month. HQPAF complements the Annual Wellness Exam and supports a variety of CMS quality programs, including The Healthcare Effectiveness Data and Information Set (HEDIS) and the Five-Star Quality Rating System.
