Ten Medical Group Offices Selected for Pilot Call Me First Program

CHI Saint Joseph Health Partners, CHI Saint Joseph Medical Group and KentuckyOne Health Medical Group – Louisville Region are collaborating on a new campaign to reduce potentially avoidable emergency room visits within our patient population, helping patients receive the right care, at the right time and in the right place. Ten provider offices have been selected to participate in a pilot program, Call Me First. The pilot program will kick off on February 4, with a roll-out to all CHI Saint Joseph Medical Group and KentuckyOne Health Medical Group – Louisville Region primary care providers beginning April 1.

Emergency Room visits are costly. Some visits are preventable and can be an indication of poor care management, inadequate access to care or poor choices on the part of the patients. Participation in the Call Me First campaign is a way for us to educate high emergency room utilizers about the role of the primary care provider in coordinating ALL of their care, and to educate patients on the locations of nearby after-hours urgent care centers.

The Call Me First campaign will include educational information for our staff, employees and patients, as well as flyers and website postings. Additional detail will be shared with provider offices and facilities as the program is rolled out. Not only will this effort provide better health and care, it will improve performance in our shared savings contracts.

The following medical offices have been selected to participate in the pilot program:

3. CHI Saint Joseph Medical Group, Whirlaway Drive, Danville
4. CHI Saint Joseph Medical Group, Monarch Street, Lexington
5. CHI Saint Joseph Medical Group, Harrodsburg Road, Lexington
6. KentuckyOne Medical Group – Louisville Region, Southend Medical/Dixie Highway, Louisville
7. KentuckyOne Medical Group – Louisville Region, Valley Medical/Dixie Highway, Louisville
8. KentuckyOne Medical Group – Louisville Region, Bardstown Road, Louisville
9. KentuckyOne Medical Group – Louisville Region, Mack Walters Drive, Shelbyville
10. KentuckyOne Medical Group – Louisville Region, Hebron Lane, Shepherdsville

How Can You Help?
Even if your practice or department isn’t heavily engaged in the Call Me First effort, we all have a role to play. Watch for more information about the campaign and post materials in your office. Have a conversation with your patients about when to utilize the emergency room and how to be access after-hours care. EVERY team member can play a role in helping our patients and co-workers prevent unnecessary ER visits.
